Use this form to request a direct rollover from another qualified 529 plan or education savings account to a PA 529 GSP account.

Use this form to make additional contributions to a PA 529 GSP account. ( Do this online.)

Use this form to start, change or stop automatic bank transfer contributions, or to add or change bank information for electronic contributions to a PA 529 GSP account. ( Do this online.)

Use this form to change your name, address, phone number, account owner, beneficiary, successor account owner information, or to add an interested party to a PA 529 GSP account. ( Do this online.)

Use this form if your employer offers payroll deduction and you want to have money automatically deducted from your paycheck and deposited into a PA 529 GSP account, or if you want to change your existing deduction. ( Do this online.)(If you're opening a new account, you can enroll online and select payroll deduction as your contribution method. Or include your payroll deduction instructions on the Enrollment Form.)
