    Use this form to start, change or stop automatic bank transfer contributions, or to add or change bank information for electronic contributions to a PA 529 GSP account. (Do this online.)
  • Services for Your Account Form
    Use this form to change your name, address, phone number, account owner, beneficiary, successor account owner information, or to add an interested party to a PA 529 GSP account. (Do this online.)
  • Payroll Deduction Instruction Form
    Use this form if your employer offers payroll deduction and you want to have money automatically deducted from your paycheck and deposited into a PA 529 GSP account, or if you want to change your existing deduction. (Do this online.) (If you're opening a new account, you can enroll online and select payroll deduction as your contribution method. Or include your payroll deduction instructions on the Enrollment Form.)
  • Form W-9, Request for Taxpayer Identification Number and Certification
    Use this form to certify the account owner's or beneficiary's taxpayer identification number.
  • Limited Power of Attorney/Agent Authorization Form
    Use this form to grant another person or organization limited authority to act on your PA 529 GSP account(s). This means that the person or organization may receive certain account information or take certain actions that you specify on the form.
  • Power of Attorney Form
    Use this form to grant another person or organization complete authority to act on your PA 529 GSP account(s). This means that the person or organization may receive any account information or take any action that you, as the owner of the account, could receive or take.
  • Organization Resolution Form
    Organizations designated as an agent on a Power of Attorney form or a Limited Power of Attorney/Agent Authorization form must use this form to identify the individual(s) who is authorized to receive information or take authorized action on PA 529 GSP account(s) on behalf of that organization.

Investment Plan (IP) Forms

The Pennsylvania 529 College Savings Program sponsors two plans - the PA 529 Guaranteed Savings Plan (GSP) and the PA 529 Investment Plan (IP). The guarantee of the PA 529 Guaranteed Savings Plan is an obligation of the GSP Fund, not the Commonwealth of Pennsylvania or any state agency. Before investing in either plan, please carefully read that plan's disclosure statement to learn more about that plan including investment objectives, risks, fees, and tax implications.

If you are not a Pennsylvania taxpayer, consider before investing whether your or the designated beneficiary's home state offers any state tax or other benefits that are only available for investments in that state's qualified tuition program.
